Programmes including the federal Broadband Technology Opportunities Program funded regional middle-mile construction specifically to remove this bottleneck. The Vast Networks regional backbone grew out of one such project, reaching across 18 California counties, which is why the network reaches places commercial economics alone would not have justified.
A provider with its own middle mile can reach outlying sites and price regional transport without buying it from someone else. For an organization with facilities away from metro areas, that is the difference between a viable quote and a polite decline.
Two access circuits into a building can look diverse and still converge onto the same middle-mile route a few miles away, which means one backhoe takes out both. Genuine diversity has to be established along the whole path, not just the last stretch, and the honest answer for a given pair of sites is sometimes that it does not exist at a sensible cost.
